Health and medical related information that are relevant and necessary to your physiotherapy assessment and treatment.
Details of other health and social care professionals that you may be involved with.
How do we collect your data?
We collect data regarding your name and contact details when you make a telephone or email enquiry or book a physiotherapy appointment.
We collect personal health and medical information and details of other professionals involved at the time of your initial physiotherapy consultation and as relevant at the time of any further follow up appointments.
Deepdale Therapies may also receive your data indirectly from a referrer or other health or social care professional only if you have already provided consent for this to occur.
Why do we collect your data?
Deepdale Therapies collects your data so that we have an accurate legal record of our interventions with you in line with the health and care professions council (HCPC) standards of conduct, performance and ethics.
We will only share your data with other health and social care professionals if you have agreed and consented to this only for the purpose of managing your health.
Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:
(a) Your consent. You are able to remove your consent at any time. You can do this by contacting us using the details above.
(b) We have a legal obligation.
How do we store your personal information?
Deepdale Therapies securely stores your personal information, and health and medical related data electronically using allied health practice management software.
We store your data for 8 years in line with HCPC legal requirements for health related records. Once this time period has expired, your data will be destroyed securely.
